Become a MyLLT Member Company

 Apply only if you meet the criteria below:

• Company mission statement aligns with the mission of MyLLT. 
• Company is a US based company and agrees with the MyLLTsubscription Terms of Service.
• Company wishes to add a MyLLTsubscription as an employee benefit. Employees subscribe through an appstore using a discount code.

 Submit completed application packet with annual membership fee

• Use the Request Application form above to request a Member Company packet.
• Submit completed packet with annual membership fee. 

            $240 featured on Member Company page or $120 no feature. 

• Allow 14 business days for processing. 
• Check your email for status updates, requests for more info, and final decision. The annual membership fee will be refunded immediately if declined. 

 Plan for next steps after approval

• Upon approval we'll create an implementation timeline. 
• At your request we'll provide you a sample communication packet. Company is responsible for all functions and cost related to embedding the product and all communication to your eligible population.
